Damaged Passports
Passports that are damaged or in poor condition should be replaced, regardless of remaining validity. For security reasons, damaged passports may be retained by this office. Please note that your damaged passport is not proof of citizenship or identity. You will be required to present either evidence of citizenship or appropriate identification. If you are not able to do so, please contact this office for further information.

Please review the following SIX STEPS before submitting your application.
Step 1 - Complete the form DS-11, Application for a U.S. Passport
Applicants applying in person must complete Passport application form DS-11. Applicants aged 16 and over must sign their own application. Please ignore pages one and two of the printed instruction pages, as they do not apply when applying for your passport in the United Kingdom. Follow the instructions on this page. Form DS-11 must be printed on separate sheets of paper; do NOT print this form on front and back of the same sheet. Step 2 - Present evidence of U.S. Citizenship
When applying for a U.S. passport in person, applicants must present evidence of U.S. Citizenship with Form DS-11. All documentation furnished as evidence will be returned to applicants.
Applicants should be prepared to furnish one of the following:
- a U.S. Birth Certificate;
- a U.S. Consular Report of Birth Abroad;
- a U.S. Certification of Birth;
- a U.S. Certificate of Citizenship;
- a U.S. Naturalization Certificate;
- a passport of your U.S. citizen parent(s) in which you are included; or
- your original, long-form birth certificate, if your proof of citizenship is the U.S. passport of a parent in which you are included.

Step 3 - Present proof of identity
When applying for a U.S passport in person, acceptable identification must be presented at the time of application. Acceptable identification includes:
- a valid passport of any country;
- a valid driver's license with photograph;
- a national identity card with photograph;
- a valid U.S. or State government ID card; or
- a U.S. military ID card.
Step 4 - Pay the applicable fee
The application fee for an applicant under the age of 16 is U.S. $105.00; aged 16 and over is $135.00. The fee is paid on the day of the appointment. Accepted payment methods are:
- Credit card - Visa, MasterCard, American Express, Discover, or Diners Club, or Debit card - Visa or Mastercard only;
- International Money Order to the exact U.S. dollar amount made payable to the "United States Disbursing Officer";
- Banker's Draft drawn in U.S. dollars only on a bank in the United States made payable to the "United States Disbursing Officer"; or
- If applying for the service in either London or Belfast, cash - either dollars of sterling equivalent
Please note: We are not authorized to accept personal checks. Money Orders or Banker's Drafts must have been issued within the last five months. The fee is a non-refundable processing fee and is retained by the Department of State whether or not the passport is issued.
Step 5 - Passport Photographs
Applicants must provide one passport photograph: We will reject any photographs that do not meet the specified requirements. Vending machine photographs and photographs printed on a home computer are unacceptable.
Step 6 - Scheduling the Appointment
You are required to apply for the passport in person through a pre-arranged appointment. On scheduling an appointment, you will receive a Confirmation Sheet. Please print this page and bring it with you to the interview.
