Name Changes by Means Other Than Marriage
 

If you have changed your name, you may apply for a new passport to be issued your new name. Under U.S. passport law, only certain forms of name changes may be entered into a U.S. passport, and application requirements vary depending on the way the name was changed.

Before You Start, Please Note:

Please review the following FIVE STEPS before beginning your application.

Step 1 - Complete form DS-5504, Application for a U.S. Passport

Applicants must complete the passport application form DS-5504. Parents should sign for children under age 16. Please ignore pages one and two of the printed instruction pages, as they do not apply when applying for your passport in the United Kingdom. Follow the instructions on this page. Form DS-5504 must be printed on separate sheets of paper; do NOT print this form on front and back of the same sheet. Step 2 - Additional Documents

  • Your current U.S. passport; and
  • The documents as requested for your specific type of name change.

Step 3 - Fees

There is no fee for this service.

Step 4 - Passport Photographs

Applicants must provide two identical passport photographs: The Embassy will reject any photographs that do not meet the specified requirements. Vending machine photographs and photographs printed on a home computer are unacceptable.

Step 5 - Scheduling the Appointment

You are required to apply for the passport in person through a pre-arranged appointment.   On scheduling an appointment, you will receive a Confirmation Sheet.  Please print this page and bring it with you to the interview.

Additional Requirements for Passports Issued More than 12 Months Ago

In addition to the above listed requirements, please also provide the following if your passport is more than one year old:

  • Passport application form DS-11 instead of form DS-5504: Applicants aged 16 and over must sign their own application. Please ignore pages one and two of the printed instruction pages, as they do not apply when applying for your passport in the United Kingdom. Follow the instructions on this page. Form DS-11 must be printed on separate sheets of paper; do NOT print this form on front and back of the same sheet.   • Fees: The application fee for an applicant under the age of 16 is U.S. $105.00; aged 16 and over is $135.00.  Accepted payment methods are:
    • Cash - either dollars or sterling;
    • Credit card - Visa, MasterCard, American Express, Discover, or Diners Club;
    • International Money Order to the exact U.S. dollar amount made payable to the "United States Disbursing Officer;" or
    • Banker's Draft drawn in U.S. dollars only on a bank in the United States made payable to the "United States Disbursing Officer.

Note: The Embassy is not authorized to accept personal checks. Money Orders or Banker's Drafts must have been issued within the last five months.
