Please review the following SIX STEPS before beginning your application.

Before You Start, please

  • Review the information in the "Three Most Forgotten Items"box in the upper right hand corner of this page.

Step 1 - Complete form DS-11, Application for a U.S. Passport:

Applicants applying in person must complete the passport application form DS-11.  Please ignore pages one and two of the printed instruction pages, as they do not apply when applying for your passport in the United Kingdom. Follow the instructions on this page. Form DS-11 must be printed on separate sheets of paper; do NOT print this form on front and back of the same sheet. If you experience trouble with the Passport Application Wizard, click here for Form DS-11 [PDF - 1,004kb].

TIPS for applicants applying in the United Kingdom is available here

Note: If you are applying for the first passport for a child under the age of 18 who was born overseas and the birth has not been registered with a U.S. Embassy or Consulate, you must report the child's birth before applying for the passport.  You may apply for the child's passport at the same time you report the birth.
