Filing for Benefits or Medicare
 

All claims for Social Security benefits or MediCare are taken by scheduled telephone appointments. 

The link is at the end of the page BUT NOTE THAT you will be asked to provide the following information:

  1. Your full name
  2. The last 4 digits of your Social Security Number (SSN)
  3. Your date of birth
  4. Your MAILING address - not your U.S. address
  5. Your telephone number(s) that we use to reach you during business hours
  6. Your email address
  7. If you are married we also need:
    1. Full name of your spouse;
    2. Your spouse's date of birth;
    3. The last 4 digits of your spouse's social security number IF they have one;
  8. We will need to know if you are unable to work because of a disability
  9. We will need to know if you are working more than 45 hours per month
  10. Please also provide you UK National Insurance (NI) number
