Passport Applicants Under the Age of 16

Applicants under the age of 16 must appear at the Consulate accompanied by both parents. If only one parent is able to attend follow the "Application by One Parent" instructions given below. The application must be made on form DS-11 and additional documentation, as described below, will be required.

Passport renewals for children whose most recent passport was issued under the age of 5 should present photo age progression from the age in the most recent passport to the present age.


Proof of parentage. You must submit one of the following:

  • a long form birth certificate listing both parents' names;
  • a Consular Report of Birth Abroad or Certification of Birth Abroad listing both parents' names ;
  • adoption decree, with adopting parents' names;
  • court order establishing guardianship or custody.

If the parents' or guardian's names are different from that shown on these documents evidence of a legal name change must be provided.

Proof of parents' identity. Each parent or guardian must submit one of the following:

  • valid U.S. or foreign passport with recognizable photo;
  • valid driver's license;
  • valid U.S. state, military or government ID;
  • U.S. naturalization or citizenship certificate with recognizable photo;
  • U.S. alien resident card.

Application by one parent

If only one parent is able to appear, one of the following will be required in addition to the documents listed above:

  • Statement of Consent (Form DS-3053) completed by the absent parent must be notarized before a U.S. notary public, British notary public or British solicitor. The notary or solicitor must place their official seal on the form. If the absent parent is in a third country, not the United States or United Kingdom, they must take the Statement of Consent to an American embassy or consulate for notarization. The Statement must be accompanied by proof if identity as listed above;
  • Evidence the appearing parent has the sole authority to apply for a passport. For example, a birth certificate or report of birth abroad listing only one parent, an adoption decree listing only one parent, court order granting sole custody, court order specifically permitting the applying parent to apply for a passport, judicial declaration of incompetence of the absent parent or the absent parent's death certificate.
